Lower eyelid blepharoplasty is performed to address the lower eyelid bags or hollows. In 2022, the treatment for lower eyelid bags and hollows is different than it was a decade ago. There are many plastic surgeons that simply remove lower eyelid fat through an incision in the lower eyelid in everyone causing that hollow, sculpted, surgerized, look that was commonly seen in the early 90s. Over the last decade, the advances in hyaluronic acid fillers have helped defer or avoid surgery in many people. There are, however, many people who still need lower eyelid surgery to address those bags under the eyelid.
We are the primary referral practice for med-spa and aesthetic practices in the San Diego area for lower eyelid blepharoplasty. Our practice regularly sees patients who have undergone lower eyelid blepharoplasty by a general plastic surgeon and now need repeat surgery to fix a suboptimal result. As a fellowship-trained oculoplastic surgeon, Dr. Iyengar brings the highest level of expertise to the periocular area. Also, an expert in lower eyelid rejuvenation and with a decade of experience in lower eyelid fillers, Dr. Iyengar is able to distinguish who might benefit from fillers and who might benefit from surgery.

This nice woman had lower eyelid blepharoplasty performed by a San Diego general plastic surgeon who is not an eyelid specialist (oculoplastic surgeon). She was disappointed with the outcome and came to see Dr. Iyengar for revisional eyelid surgery. There are three lower fat pads in the lower eyelid that generally cause the lower eyelid bags. In this case, the fat pad near the nose, known as the nasal fat pad, was missed during eyelid surgery. The original eyelid surgery was performed with an incision under the eyelashes which cut through the orbicularis and the reason her left lower eyelid pulls down. Dr. Iyengar was able to fix her suboptimal result using a scarless approach from the inside of the eyelid. This is another example of why it is important to see an oculoplastic surgeon for your eyelid surgery.
